• Both the new patient and the referring patient must complete the required online referral form to be eligible.
• Limit one $50 Visa® Gift Card per new patient and one $50 Visa® Gift Card per referring patient per referral.
• Referral reward is issued only after the new patient completes their appointment.
• Gift cards are distributed digitally via email within a reasonable processing period after the visit.
• No cash value. Gift cards may not be transferred, substituted, or redeemed for cash.
• Offer cannot be combined with other promotions, discounts, or insurance-based incentives unless otherwise stated.
• Practice reserves the right to verify eligibility and deny or revoke rewards in cases of fraud, abuse, or failure to meet requirements.
