New Patients:

• Must schedule their appointment and be seen during the month of October for entry into our Grocery Giveaway.
• You can schedule your appointment online via the form at the bottom of our page OR call your local office to schedule (mentioning the special!)
• All New Patients will get one entry into our Grocery Giveaway and will also receive a $10 Starbucks gift card at their first appointment.
• Each New Patient under 18 years old will count as one entry and will add to their Legal Guardian’s Entry total.

Existing Patients:

General Rules: Contest is open to the general public in North Carolina who are patients at Lane & Associates or NCOSO, except for employees, agents, contractors, subsidiaries and affiliates of Lane & Associates Family Dentistry and NC Oral Surgery + Orthodontics, participating sponsors, their immediate families, and household members. All participants must be 18 years or older to be eligible to win or claim the prize but a parent can enter on behalf of a child. One Entry per patient. There is no household limit. Offer is valid for New or Existing Patients only. Mention this promotion when making your appointment. Not valid with any other discounts or promotions. Void where prohibited by law. Other restrictions may apply. Prize is valued at $5,200. All prizes are awarded “as is.” Prizes are non-transferable and cannot be exchanged for other trips, prizes, or cash. The winner will be required to use the gift certificate in Grocery Store locations for any purchase OTHER than lottery tickets. Lane and Associates reserves the right to change the contest rules or dates at any time. *Offer not available to any recipient of Federal Health Insurance including Medicaid and Medicare as mandated by Federal Statute 42 U.S.C. 1320a-7b(b).
